I agree to release and absolve Garden City Productions, its management, members and representatives of any and all responsibility and liability for any personal injury, loss and/or damage to personal property that may occur as a result of my participation in its activities both on and off stage.

I consent to the use by Garden City Productions of any photographs or videotape containing my voice and/or image for the purpose of promoting GCP’s financial position and I agree to make no monetary or legal claim against GCP for such use.
